Management Referral

We’ll help you address difficult situations

Problems with individual employees can negatively impact moral, job performance and your bottom line. Cascade Health offers supervisors the opportunity to refer employees for disciplinary and performance management issues to better the workplace.

While the majority of management referrals are resolved within a few sessions, we will provide as many meetings as needed. Download our referral forms below.

Supervisor referral steps

  1. Recognize the problem.
  2. Complete applicable forms and document the problem.
  3. Fax forms to DIRECTION.
  4. Have the employee call to schedule the first appointment.
  5. The counselor will call prior to the first session.


Supervisors normally consult with a counselor regarding the employee issue either over the telephone or in person at the DIRECTION for Employee Assistance office. Prior to making a referral, the supervisor talks with the employee and defines the desired behavioral change and have he or she call DIRECTION for Employee Assistance services.

Release of information

The employee signs the management referral form. Then the employer provides written documentation outlining the problem. This allows the counselor to review the issues prior to meeting with the employee. During this initial meeting, we carefully outline the management referral procedures answer any questions the employee might have.

Contact, communication

Our counselor contacts the supervisor to report that the employee has made contact, and followed through or did not follow through with counseling. Regular feedback is provided to the employer until the problem is resolved or the case is closed.

Group sessions available

In addition to or in lieu of one-on-one counseling, meetings can also be arranged to include the supervisor, counselor, and the employee.


Making an employee referral is easy

We’ve provided the following forms to assist you in the employee referral process. After you have contacted and spoken with a DIRECTION for Employee Assistance counselor, simply download, print, and complete the applicable forms, then fax them to 541-345-4419.
